How important was ethnicity in Ancient Rome?

Was there any idea of "blood purity" in the sense that some people in the empire were less Roman than others, or was your status decided mainly by culture?

Depends on the period but they didn't have an idea of blood purity. But to be a Roman you did have to be somehow related to Central Italy, even if you just made it up, with Rome being the most Roman-Roman somebody can be when tracing their lineage. In the Middle Republic they had an idea of an Italian race, which was made up of the peoples of Central Italy and the Romans were part of it, it's what the Gracchi argued in defense of, that they wanted to strengthen the Italian race by giving them fair amounts of public land. And this already shared idea of a race made it easier for these people to become 'Roman' as they already had an idea of a shared lineage and culture.

Later on, people frequently tried to tie regions to Italy in some way to legitimise how Roman they really were. We have surviving legends recorded by Constantine VII about why they are actually super duper Roman. In Dalmatia the justifies why they call themselves 'Romani' as according to them, their cities were refounded by Diocletian who sent the best families from Rome there, making them super-Roman. This is a later reasoning but it is still quite similar to the justifications they used in Antiquity. That they are somehow tied to Italy by some ancient ancestry, so that they too are part of the Italian race. This props up when regions essentially adopted Roman culture and Latin and by Late Antiquity even in the East. Which gives you some insane reasoning by Roman authors like the Jews actually being from Italy originally and migrated to the Levant, meaning the Jesus was actually an Italian all along.

In summary, yes the Med-MENAs were obsessed with "muh purity" on many levels. The real question is "Why?". They all look alike and none of them looked good to begin with. Then again, you can't work with what you don't have ;)

Extremely. Up until the late republic, anyone born outside of the immediate roman (the city) countryside was considered ethnically lesser, to the point where even the likes of Cicero and Marius saw discrimination
After the social war, this gradually lessened for latins first and then other italics due to a multitude of factors, like complete cultural assimilation, the loss of prestige of roman urban politics, the effective integration of the whole italic upper class into equites roles like the tres militiae, etc.
Aside from actual italics and the least mixed colonials however nobody else really manage to break into ethnic romanity after that point.
Of course that didn't really matter anymore since by the mid-empire romanization had unified the empire at the cultural level, making ethnic distinctions not that socially important anymore. A non-italic citizen would easily be able to reach any position he wanted.
