How and why did the understanding of sex/gender go from a combination of inherent biology + socialization (a woman is a female who was raised a girl, a man is a male who was raised a boy) - with sex and gender being basically synonymous terms - to whatever the fuck is going on in modern western culture?

Buddhism (with it's not-self attitude toward the five aggregates, one of which is the body) and a good portion of Christanity (though moreso the heretical varieties) would disagree I think. There have been many different iterations of the same cycle including the decadence phase, no doubt there have been many self-erasing cultural moments. If there's anything unique about today it has to start with technology, a secular understanding of the world that would be impossible before modern science. The difference between Buddha or Jesus and LGBT is they believed in a distinction between the material and spirit worlds. Even where doctrine relates to the physical body, it would never declare supremacy over the physical because that is neither needed nor indicated. People become eunuchs for the kingdom of heaven, not to fix a problem with their bodies. Only today does the battle for meaning take place on physical terms, an impossibility before technology permitted communication at a distance. In the past, arguments over biology and physical reality had physical answers. Today, people still need to talk about the soul and the ineffable but they've lost the language.
