The photo shows a Filipino survivor of the Manila Massacre (Feb 3–Mar 3, 1945) during the Battle of Manila in WWII. Japanese marines under Rear Adm. Sanji Iwabuchi killed at least 100,000 civilians via mass executions, bayoneting, beheadings, rapes, and burnings in hospitals, churches, and streets as Allied forces advanced. This scar on the neck is from a Japanese officer's saber slash in a failed beheading attempt.
