it starts with making material restrictions in your environment, such as putting certain temptations away (if only temporarily), blocking sites, etc. then having physical aids to serve as reminders (sticky notes, alarm clocks, etc.). also mindfulness (which can be facilitated by meditation, but also just thinking long-term).
