Not usually, I travel a lot for work and just go to different places I've never been in before. If I do tip I just leave a five. It's the same amount of work to carry a $10 meal and a $100 meal so I don't see why it should be a scaling price. The server didn't make the food. All they have to do is bus dishes, be polite, and leave me alone while I eat. It isn't a difficult or dangerous job.

No, it's not in our culture and you can't even do that anyway. They wouldn't know what to do with it, and couldn't even take it since that's overpaying and would mess up the bookkeeping and get the place shut down or something.
