In the old times building techniques, skills and materials were rare and usually became localized which caused all the buildings to be built the same way from the same materials. Somewhat true. Cities would be surrounded by farmsteads but in ye olden days the city walls weren't just fortifications as they were also the physical demarcation of the city and its privleges. If you lived outside the walls you weren't a citizen. And often there were statutes that limited the expansion of cities as those stood in a direct competition with the secular and ecclesiastical princes, who naturally wanted most of the available land for themselves.
Only in the 18th century and following did cities outgrow their walls as militarily they lost most of their value and political changes began to favor cities again.
