Interesting, definitely a shift from the Gensokyo set in the relatively temperate Japan. Makes me wonder how life there would look with a yearly mean temperature of 1.8 degrees celsius (compare Japan's 11 degrees), especially if they're a few hundred years in the past, like Gensokyo. There really might be a case for moving to Makai, if that's the case.

The equivalent human village culture would be really interesting, given that it'd probably be very viking adjacent and as such would probably maintain a more aggressive, warrior-esque culture than Gensokyo's human village. We might even see them as an actual faction, with humans causing incidents themselves.

Personality if I had to find a place for a European Gensokyou then it would either be Iceland or Wales. Iceland was already explained by you but Wales already has mythology surrounding other realms (Specifically Fae ones) and a big chunk of cool British folklore and myth come from Wales (Like King Arthur). Plus with Britain being a melting pot of various european cultures (Celts -> Romans -> Anglo Saxon -> Viking -> Normans etc.) it would make Wales the perfect place for all kinds of european gods, monsers and other creatures to gather.

That would be really damn cool, actually. You could have the western, germanic deities (Wotan, Thor, Tyr) and the eastern, finnish / baltic ones (Ukko, Perkele, Tursas) come into conflict, along with their respective gothi / shamans (who might become playable characters?). I just really want a qt shaman 'hu that wears a deer skull on her head.

For a third faction, you could even throw in the slavic pantheon (Perun, Svarog, Dazhbog), if you want to keep it based and hypothermiapilled. I feel like this version of Gensokyo could lean harder on grimsokyo and get away with it, because it just makes sense. I wouldn't mind seeing some ritual sacrifices or blood being drunk here or there.

Medicine is probably Amerigan actually
Before WW2 America send a lot of "blue-eeyd" dolls as diplomatic gifts for Japan, they were an instant hit among children for their exotic appearance, notice how a lot of blonde Touhou characters have varying eye colors, but Medicine's are constantly blue, it's probably intentional as blue eyes is heavily associated with western dolls in general.
Since PoFV ultimately goes back to WW2, during the war it was common for these dolls to be discarded as they were seen as unpatriotic, often without the proper ways you should "bury" a doll in Japanese culture and iirc Medicine's grudge came from being disregarded as a tool, the symbolisms of the Lily-of-the-valley, her color scheme heavily associated with humorism, and the power to manipulate poison further enhance that possibility, while it's not clear when Medicine came to being, it was probably roughly around the time of PoFV (so near the renewal of nature) also note how the violet cherry blossoms at Muenzuka cling on Medicine, they are souls who died without even realizing it (victims of the bombings) perhaps some were children who had a doll just like Medicine was.
But that's just my personal reading
