Gensokyo is never really changed, only expanded upon such that derivative works never feel antiquated. If Akyuu were to die, it'd leave those who want to use her character in an awkward predicament. The setting would be split in two: a "pre-death" and "post-death" Gensokyo. It is because of the lack of such major events that ZUN is able to play fast and loose with chronology outside of story arcs.Also, Akyuu's passing would make Kosuzu sad. It was just supposed to be a detail to establish Akyuu as distinctly magical and extraordinary, coupled with characterizing her as self-sacrificing and always working towards the good of others (and back in PMiSS Gensokyo was still written as a mostly worthwhile place for humans to be, largely open towards youkai and having opportunities not found in the outside world).
