Even if the entire crew of a nuclear silo or submarine went rogue for a year, they wouldn't be able to bypass the PAL to arm the warhead without an EAM. Only in the 40s-50s would localised action be possible before governments realised how insane that situation was.

You're seeing a dramatization of a missile launch, giving the impression that it's just a couple keystrokes away on a (faulty) computer. This is not the case at all; a machine like that needs the consent of many operators, commanders, and technicians in order for a successful launch to happen. You'd need to compromise a lot of people, which is something I'm sure there exist counterprocedures for.
You could argue that even a legitimate order to launch would face such problems. I don't know how one would actually convince a crew to follow through on these orders, but fortunately it's not a situation we've had to face... yet.

Nukes like the W-88 have the peanut shape for a reason. Even if you could detonate some of the explosive lenses, the shape is designed to facilitate a "toothpaste squeeze" effect where you basically create two sub-critical masses in the primary squeezing into two separate spheres instead of one big peanut. That being said, it definitely appears to be a possibility. Two of the Sigma clearance levels within the known DoE classification structure specifically address "knowledge to facilitate the unauthorized detonation of a nuclear weapon". The Sum of All Fears (book, not retarded movie) is pretty great because it addresses this specific scenario.
