As a righty your barrel/slide would be mostly in line with your left inguinal area instead of your right like it would be with appendix. Your grip is going to the right though like it would with standard appendix. You'll want to slide your belt buckle off center a bit to avoid it being in the way or adding to girth. I do this because while I may be a lot of things but fat isn't one of them. That area has the most "drape" from your shirt and conceals the grip the best.

It's just low enough so that you can have you arms relaxed at you sides without having your holster feeling like it's in the way; it's out of the way enough so that you can do all your normal shit without it feeling in the way.
