why did iron fist APS never take off?

Originally it was not very good in US testing. Supposedly it has improved substantially now and from what I remember the US was looking into using it for Bradleys although that might not materialize. Trophy is a significantly easier system to get to work because it is essentially just a supersized shotgun instead of a missile that has to detonate at the precisely correct millisecond.

Friendly fire is a longstanding issue with APSs. Rheinmetall's ADS is arguably the least-bad but there's no real good close-range system.
Iron Fist launches a large-grenade sized explosive into areas where your infantry are highly likely to be. Trophy at least confines the danger to a narrow fan of shot.

That defeats the point of having APS since you shouldn't be using tanks in the first place if friendlies aren't nearby.

Most countries are finding it hard to justify the cost of APS if they aren't at war. So for now, only a handful of countries are getting Trophy, and only for their tanks.
Many others are getting vehicles that could take Iron Fist, but aren't putting Iron Fist on them yet.

the APS tends to fire a narrow cone in the direction of the attack
while it could theoretically cause a friendly hit, its much less likely than allowing an explosive to impact the tank and create a hemisphere of fragmentation

if the APS does trigger an RPG to go off, there really isnt a significantly increased danger from the warhead if it goes off on the tank or 10m from the tank
