The damage the Fat man does in game could be done with conventional explosives, so yes you could produce such a launcher. It would be limited as some kind of clearing weapon though most likely as basically it would be equivalent to blasting a target with a HE round.

Yes. Infantry are equipped with recoilless rifles (Carl Gustav), rocket launchers (RPG), and "handheld" mortars like the M224. It was not fully developed and implemented into service because it was not seen as an improvement over current weapons. The Fatman in standard configuration could be fired by a soldier. Turning it into a "shotgun mortar" like your picture would make it impossible for a soldier fire without getting grievously injured or killed by the recoil. Regardless these weapons are supposed to be used with Power Armor; that would dampen some of the effect.
