If its in the chamber, its game over. I think it was an incompletely closed breech and the crew died from overpressure. The 4th crew member was probably standing up in a hatch and got ejected or was not inside the tank. The old american 175 mm self propelled field gun used to suffer from exploding HE shells, it turned out that sometimes tiny bubbles would form in the HE filling and when the shell was shot off the rapid acceleration would crush these bubbles so hard that the HE would explode.
