Companies fail because executives cut corners, or finance guys broke rules designed to stop them fucking up.
A sub imploded because the boss was sure he knew better than the regulators.
Governments have fallen because the leader was sure that rules didn't apply to them.
An entire nation is collapsing before our eyes (no replies req) because the head honcho can't be restrained from following his worst impulses.
These alpha types always think that they can dodge any consequence and so they take obviously stupid risks and sometimes their luck runs out.
People get dragged down with them because they didn't or couldn't stand up to them.

It's really not specific to the military, arseholes are everywhere and if you let them into power, bad things happen.
