Transitioning is not (usually) done for fetish reasons and doesn't leave you disabled. On top of that, gender dysphoria is well-recognized and being transgender is extremely well documented and transitioning is a proven method to improve the lives of trans people.

The biggest issue here is the fact that there is literally no proof that amputating someone's legs will improve their quality of life which is what you need to show in order for something to be covered by insurance. Had he simply paid a willing surgeon to get his legs amputated nothing would have happened.
