Trans clinics opened in the 70s in the US, but speaking as an older trans person I didn't start seeing the T being put in the alphabet soup till after I transitioned. So like in the 2000s at my uni the LGB club was, well, just the LGB.

It was later in the mid 20th century with the work of clinicans such as Harry Benejamin do we start to see the term "Transsexual" being used and gender affirming healthcare being provided. There are documentaries about the Compton town Riots you can watch you can see trans women from the sixties talk about their response to Harry Benjamin's work at the time and how they felt "understood" by the label.then we have this back and forth a bit between "Gender clinics for treatment!" and "gender clinics are an abomination" and its not really until the early internet age where the trans community really starts to organise. Transgender as a term was initially used by this one activist who wanted to differentiate herself from transsexual as term, but through the werid amorphous way language works transgender somehow became an umbrella term and transsexual became considered taboo (lgbt subcultures have this annoying habit of demonising words that were considered fine previously)So transgender as a word became commonly used in the late 2000s early 2010s. but the modern western understanding of what a "transsexual" is has roots back to the late 19th early 20th century, while gender variance is just a thing that happens cross culturallymost of this was typed from memory but trans literature is pretty much its own subfield with many books about it.
