It's unpredictable.

Generally shrinkage will either result from atrophy induced by a lack of erections (full erectile dysfunction; atrophy is permanent after not having erections for a long period of time), or from not getting as hard as you used to (partial erectile dysfunction).

If you're lucky, you won't lose erectile function to begin with, and shrinkage will not occur. If you do lose erectile function, tadalafil (5-10mg daily) can almost always bring it back. If you're unlucky, nothing will restore full erectile function and you'll either have to stop HRT or live with it.

Making sure erections happen is another matter entirely. Men on testosterone will generally get involuntary erections (nocturnal and spontaneous). Sometimes people on HRT lose one or both of these. If you do, you'll have to manually get hard a few times a week to avoid atrophy (remember, a lack of erections means atrophy).
