pioglitazone. it is traditionally a diabetes medication, but one of it's prominent side effects is redistributing fat, primarily to the trunk region. this only really works for ppl w/ lots of estrogen, so dont start it until you've been on hrt for a few years and have already noticed some fat redistribution. start w/ 15mg/day for 3-6 months at a time, with long breaks in between. i can personally vouch for it's effectiveness, my ass and hips ballooned after gaining 30lbs while taking it

Pioglitazone.
It is a PPARg agonist. The mechanics of how it works are quite complex; honscientists on this board will not give you a proper explanation. Trannies use it as a fat redistribution enhancement drug, since it encourages distribution of fat towards the lower half of the body (thighs, ass, and so on).
If HRT is like a 1/10 in terms of danger pioglitazone would be like a 5/10. The main thing you have to look out for is bone density loss, but there are a lot of other nasty risks you can look into (read the monograph).
Most folks will be able to get equivalent fat redistribution effects on just HRT, albeit over a longer period of time. Most of the conditions that would necessitate pioglitazone are things you can go to the doctor and get a legitimate prescription for.
