I want to put this as gently as possible but there is no way to explain it that doesn't come off as harsh. If you really want to know the answer to this it's autism. Not "haha I like paradox games" autism I mean like ones deep into the spectrum that could qualify as disabled. The brain that enjoys mastering speedrunning is also the type of brain that could have all emotional needs met in a digital world. For such people an anime girl pfp and she/her on their discord is going to be the extent of their transition. They may at some points look in the mirror and experience a mix of shame and dysphoria but then it's alleviated after returning to the digital cope life. On some level they might fantasize about looking like a woman but due to extreme executive dysfunction it never really happens. Even if they do get on hrt it's going to be the shitty 2mg estradiol pill+100mg spiro a day from planned parenthood tier and jack shit happens to their appearance. The online social circles they run in are super progressive because these speedrunning and kaizo communities are probably some of the friendliest on the internet, nobody would ever misgender anyone and they get to live out being a woman through their screen.

I don't really know any of them so I can't even dare to criticize, but personally, there's been too much damage to my body that no matter how much effort I put into this it doesn't reflect on my external appearance, for instance, I never grew a beard or a mustache, I barely grow any facial hair, but when I take pictures or look at myself in the mirror I can clearly see beard shadow even if there's no hair in there, it's just some weird coloration on that area after years of shaving the little hairs I grow, that makes me think they had it worse and it might be difficult to fix it.

Once I started learning to draw and doing digital art I just noticed super autistic ftms in the hobby that are like the trans man equivalent of OP pic. They have he/him pronouns and will use a male avatar but it's taking place entirely online and the transition never really happens. I'll watch a video where they start talking to their followers and I think "wait he wants to be a guy? I thought that was a woman talking". There are a lot of them in drawing or fanfic communities. I think it's more likely the same psychology as OP pic, neurodivergent and kind of just living out their gender in an internet bubble.
