That doesn't mean they *have* to. In fact as a layperson it can easily backfire and draw you further away from good practice toward enlightenment if you intentionally allow pain to be inflicted on you by refusing to relieve gender dysphoria. The pain can quite easily sabotage your development and draw you further into negative karma

I don’t think ignoring either of those things qualifies as extreme asceticism, which is what the middle path seeks to avoid. layperson life is a very different thing, they don’t have anywhere near the amount of rules to follow as a monastic, and it is exceedingly rare for one to become enlightened. it’s said that if a layperson were to become enlightened, they would immediately ordain because that’s the only move that makes sense.
