I live in a mid sized city in a blue state, and I see plenty of trans and other visibly queer people around pretty regularly. There are a bunch of cafes and bookstores that advertise as queer friendly and host events. There are clubs and meetups as well, plus a few art spaces specifically run by and for queer people.I’m guessing these wouldn’t all be a good fit for everyone, but I imagine if you put yourself out there and tried a few you’d meet some people you click with.
