Like if a straight kid in the 80s is talking to a guy kid and calls them "faggot", it's a command that the interaction be read by both parties as one where the straight kid has inherent moral superiority and the right to abuse the gay kid, because the gay kid is a "faggot". The gay kid, by the use of the word, is told how they are to convey themselves. The only way for the gay kid to respond is to either acquiesce, ignore the insult which will prompt violence (as it is a disobeyed command), or just prompt violence themselves. It's different from other insults, because most insults are clearly done to hurt the other person's feelings, but when you use a slur, the other person's feelings are really immaterial. What matters is that they obey. So when a tranny calls themselves a tranny, the reason it isn't as offensive is because it's a way of talking about the reality of being transgendered. Part of being transgendered is that you are a part of this category of "tranny" whether you like it or not. If someone says the word at you, YOU are being commanded, even though you've done nothing to this stranger. Calling yourself a tranny, then, can't quite ever mean the same thing as a straight person, who can truly other you totally, calling you a tranny. Besides, when someone says "I'm such a fucking tranny," for instance, this conveys the double experience of seeing yourself as thing which *deserves to be compelled by the slur* owing to your own self-loathing, and yet also realizing that this is not the case at all.

If you reject it, you're banking there's enough power in the situation to keep you safe, or you're willing to engage in open conflict. Making you feel bad is immaterial.
