We know that two ponies of one type can give birth to ponies of another type. What would happen if two pegasi living in Cloudsdale gave birth to an earth pony or a unicorn? They would need to leave their home because their foal cannot walk on clouds.

Wonderbolt academy was partly set up on flatten out mountain top, I feel like the most safe option would be to do the same for pegasi hospital or at the very least building a separate maternity wing.
