That's also what I'd do with a pneumatic tourniquet in theater.It's hard to stop a bleed when the vessels are between two bones, and blood loss is your primary concern. If you're not getting secondary care in 4+ hours, then you can trial a lower tourniquet (or two); you'll have better luck as clotting and vasoconstriction will have occurred.
