There is a reported 1.6 million trans people in America as of a 2015 Study. But for generosities sake, lets say that only 1 Million of those are on medication (this also doesn't account for 10 years of growth). That's $840,000,000 per year. And that'd be the middle estimate, for the higher end, it's $1,200,000,000 per year.And that is ONLY the hormone drugs.If you include medical expenses such as endocrinologists, primary care, telehealth, lab tests, check-ins, Prescription fees, pharmacy dispensing fees, or non-prescription supplements etc. etc.You're looking at an additional $239 million/year up to $1.079 billion/year. As this is much harder to estimate. If you add on psychiatric care which according to the 2025 Williams Institute out of the 1.6 Million estimated Trans individuals, they found a conservative estimate of Mental health diagnoses at: 58% (EHR study of 10,270 trans patients). Putting that at an even more conservative estimate at 30 to 40% getting psychiatric care, that's an estimate $300,000,000 year. Though I'd be loathed to add this onto the yearly estimate as it's almost impossible to get realistic data about this, but I figured I'd add it as food for thought.This also doesn't include surgeries, where ~10 to 15% of trans adults (~130,000–195,000) have had at least one gender-affirming surgery (2022 U.S. Trans Survey).
