It looks like a steel pile with a headstock welded on top (a plate would have been cast in to the concrete). It's actually pretty standard for the piles to end up in sub-optimal positions, and so there is a fair bit of tolerance allowed for. This particularly pile does looks pretty far out of position though, but presumably an engineer would have checked and approved this particular "fuck up".
