For the price of a small box of blueberries, you could go to Wendy's and get a junior bacon cheeseburger, 4 spicy chicken nuggets, a small fry, and a medium drink. This kind of pricing applies to any sort of healthy food (that tastes good enough for a majority of people to eat), in comparison to junk/processed food being exponentially cheaper. Factor in how huge this country is (making driving a necessity for many), whereas in many Asian countries you can simply walk or use public transportation, and it's easy to see how activity levels aren't even remotely comparable. Oh, and because conservatives are retarded and think paying an extra $800-1200 in taxes every year to cover universal healthcare is "a waste of money if I never end up using it", a vast majority of the country pays a ton of money in healthcare expenditures while also *avoiding* visits to doctors/specialists even though it would great improve their health. I'm pretty sure Asian countries don't have that problem. There's a million other reasons, but those 3 are a good starting point.
