Actually it’s the hyperphosphylization of TAU proteins. Tau protein are used to remodel and stabilize the cytoskeleton in neurons and as a scaffold for vesicle communication and waste management inside the cell. They use phosphorus as a signal to either form together or break down, but something happens and they can get excess phosphorus stuck to them and they instead stick to each other or themselves instead of the skeleton of the cell. Worse this conformational change can be spread to other Tau proteins. So you get these neurotoxic tau tangles that cause metabolic damage to the brain and slowly rot neurons.

I read about a study to make a dementia vaccine where the vaccine they used trained your immune system to detect and target tau proteins in the form they take when stuck together.
The treatment did what it set out to do. White blood cells patrolled the brain tearing down malformed proteins. Miraculously theyd even reach inside sick cells and pull out the bad protein chains.
The study went side ways though. The immune reaction became so strong in patients that they started attacking brains cells. It was theorized that the problem was that the patients were already such advanced dementia sufferers that the amounts of bad tau proteins caused an over reaction, and that less advanced patients wouldnt have that problem. Another alternative was not to inject a vaccine but only the antibodies, in a controlled dosage over time. Until the brain was cleaned out, and then give the proper vaccine as prevention.
But at least when i was reading about it, the study was sich a big disaster that the whole project got shelved.

I mostly work with pain neurons in fruit flies and and some in vitro cultures, not mammals but the immediate concern I have is that even if you clear out the Tau tangles there isn’t enough neuro plasticity in a mature mammal brain to repair the damage after the Critical periods of synaptic pruning. Even if you clear them out you would have big holes where they killed the brain unless you started young with the treatment
