If you mean why does open defecation happen in India, the reason is that in the countryside their infrastructure is such that they have to use outhouses. Because of the Hindu caste system which is outlawed but very much in practice (similar to how racism is), being seen doing sanitation work like maintenance on an outhouse can really impact you and your descendents' social status for generations. Cleaning up the sewers is for the lowest of the low in the caste system and makes you untouchable in the eyes of society. Really wealthy families make huge septic tanks that don't have to be emptied or worked on for generations, but most people can't afford this.

So, in places where there aren't sewer systems people would rather shit in the street than risk them and their family being marked as a shit cleaners and excluded from society.
