I remember reading about how they geolocated Bin Laden through a bluetooth tracking beacon he used to track Falcons once when he was doing falconry in Jordan or something years prior. They picked up that device beaconing in Pakistan and pinpointed his location.
That requires massive global Bluetooth sniffing and huge datasets. And that was 15 years ago.

The radio decibels in the 2.4MHz range gets drowned out quite easily from WiFi signals and cellphone towers. The environment also does a great job smothering 2.4MHz signals.

The way this is done is through corporate malware running on 4G cellphones that periodically broadcast nearby bluetooth devices to Google/Apple HQ, which is much more likely given how many brown people own globohomo cellphones, and it explains why they hate Hauwei so much.

Both Google and Apple have "find my phone" capabilities. They both constantly sniff and report other devices in range. With a mac address even amateurs can geolocate a device using OSINT sources like WiGLE. It would make sense the military has better and more thorough sources of detected devices and their known locations, and certainly can get access to Google and Apple capabilities.

This is the most likely answer since the alternative has problems (bluetooth operates in an incredibly noisy environment).

All Apple/Google devices periodically "phone home" with a table containing all nearby devices as part scanning WiFi networks and bluetooth devices.

All hardware devices have a unique MAC address identifier which ties back to a manufacturer hardware ID. Any SIM card that registers with that hardware ID is also linked with that MAC address.

So it doesn't really matter if Khomeini owned an iPhone or not, what matters is if his inner circle of friends owned these devices, and if he is in regular contact with him, they know his location by proxy.

No fancy radio equipment is needed, just cellphones that are backdoored-by-default. Also this is done in hardware, so you cannot opt out of it.
