The American system does this semi-directly with money via insurance with some exceptions for not denying emergency care no matter what. This allowed market forces to work as normal, meaning you get very short wait time, but you do have to deal with being able to pay. The actual quality of the care is also very high as the providers are competing on that.A public healthcare option manages supply with wait times. Since adding providers to the system is a permanent increase in government spending, it is often avoided so those wait times increase. Additionally since the providers don't have to deal with the market, there is less focus on quality. The benefit is the cost is pushed directly onto the consumer and the drug prices are cheaper (this may be changing a bit with the Trump deals with the pharma companies last year). Consumers not seeing the cost means they do try to always do preventative care which is good both health and overall cost wise.
