It happened during russias withdrawals in 2022. Sullivan and blinken took the threat seriously. It probably influenced on what weaponry ukraine received. At the time there were people who thought ukraine was winning too hard.
They aren't going to nook because part of the motivation for the war is that ukrainians are just misguided russians that need to be corrected. From their point of view it would be like nuking your own people.

We've seen how much their conventional arms, that have actually needed using in recent memory, have degraded from neglect and corruption. Even our own nuclear weapon management has huge issues because they're ultimately never supposed to be used beyond as a threat so it's kinda a career dead-end that doesn't get a lot of oversight and a lot of shit is allowed to slip through the cracks.
Russia has also lost bombers and even subs that they would use to deliver/launch nukes and perhaps most importantly they KEEP just SPECTACULARLY failing missile test launches. At least one reportedly just blew up at launch but there's video of the, perhaps more terrifying, case of the missile going up... and then turning and coming right back down nearby as those videoing freak out.
If Russia actually tried to launch a nuclear strike, especially suddenly in desperation, I honestly believe they'd most likely just end up nuking themselves.

Secondly they don't dare risk bringing NATO more directly into the conflict if a hint of radiation so much as inches past a border.
It's why they drew so many "nuclear red lines" which they allowed to be crossed with impunity that they stopped drawing them to not just keeping being humiliated.
It's why the "potential for NATO missiles on Russia's border" was an absolutely unacceptable possible security risk justifying the invasion of non-nato-member Ukraine to stop but when finland actually joined, with a border even closer to moscow, suddenly it was all about the terrain for a land invasion(as if that has gone so well for them in Ukraine lol) or something and completely different and while still SUPER bad, nato BAD!, is not demanding of any actual action.
