The real deal is that the ref is mostly to blame, with some caveats.
He rushed the count pace and was counting without checking if the shoulders were down and just presumed they were under the hair and then for some reason called the ruling to the bell without disputing his own count - which if you know how this show works is strange if they had a preset result in a title match. Was it planned after all?
Now, if it's botched then they gonna have to figure out how to deal with hair obstructing the view.

Anyway, NXT fans loved this little surprise for Thea, it's exactly the level of title she should hold for a bit after a long time and having that growing up out of Chase U angle. Monroe's been there what half a year, less? She signed 3-4 years and gets to hold titles later as she's getting over as a character. Who really cares, it's not the end of the world
