I feel like it mostly signals poor health, but that's an effect that takes eveything else into account too, so if you're not too pale or too fat or skinny or some such it's ok. I don't know if my eyes are sunken, but it's recessed enough that it really exaggerates me as looking skinny and sick and weak. Anyway, you don't have to worry about it. It does give your gaze a gravitas.
