I look like a depressed and sleep-deprived, poor uncle when I don't shave and neglect my skincare regimen. I shave, and watch my skincare for a few days, and I hit more modelesque standards. Granted, my facial structure and features are good, but it doesn't matter in the slightest if you throw in absolutely zero effort into self care; you'll still look homeless and ill. I won't pretend there are people with genuinely disfigured appearances, but tons of people don't realize how much appeal they're squandering from self-neglect. Hell, even a goofy-faced bastard can clean up and be 'presentable'.
