Curiously despite scale and disregard about safety (especially in the past) I don't know about single death case. There were plenty of smashed wrists and cracked skulls but no dead. There were several intended murder attempts, stabs into openings (face and armpits) when it's specifically prohibited by rules (personal grudges) but they survived. It's like God guards little children and retards.
