Phantasmagoria of Flower View. It's set 60 years after the end of World War 2 and the game is kind of about how all those people died and entered the afterlife at the same time. I really love the song Touhou Judgement in the Sixtieth Year. It feels sad but also transcendental and makes me think about reincarnation.

If we're talking about OSTs, I think 7.5 is a clear contendant. There are some songs NOT composed by ZUN, which give a melancholic aura unmatched in most main games, really. I personally find The End of Summer to be unforgettable. Some of Hifuu Club albums can get quite haunting, too. There are two I can think of right now: "The Purest Sky and Sea" and "Blue Sea of 53 Minutes".
Then, from the main games, I would recommend "The Perpetual Snow of Komakusa Blossoms", and both Chimata themes: "Lunar Rainbow" and "Where is that Bustling Marketplace". From th 17, "A Grief Only Known by Jizou" is fantastic.
How could I forget one of my favorite songs from ZUN ever: Faith is for the Transient People. At least I hear very sad tones to it.
Some of the ending themes are quite beautiful, too.
