It's likely a lot more nuances than than. The NFL helmets encourages players to use themselves as missiles and the hits are bigger. I'm sure there's some underreporting going on, too. Guys want to go out there and play. On the other hand between the longer seasons internationals rugby athletes play way more games. They also have less subs and more game time. I don't know enough to know if underreporting is a big thing in rugby.

Should favor the nation most times, unless it's like Japan vs a NZ club for example

Uncommon, club team qualities are a class below the national sides after all. The Barbarians (invitational club comprising mostly of celebrated international players) however do beat test teams often enough
