How come no one cares about the steroids professional athletes use? Sure they sometimes suspend guys for it but everyone is on gear. Do we just accept it's a part of having a high skill level?

Where do you draw the line for when a performance enhancing drug is too much?
Creatine, vitamins? Are those over the line? Smelling salts?
The only reason they aren't taking literal meth before a game starts is because the trade off of performance versus long term health isn't worth it, you're better off on hGH than meth so that's what they'll do

People do care, that's why there are rules. However the financial incentive to do so is overwhelming. Sporting agencies make it so the cost to do so is pretty high and is "enough" of a deterrent. There's a cat and mouse game of "don't take meds from this list and it's okay" forcing people to search for new drugs (expensive and legally tricky). By the time those drugs have made their rounds, they've been added to the list.

For the most part it's using small enough doses at specific times so that even if you get caught you can explain it away because the visible amounts will be almost non-existent (examples: Sinner and all Norwegian athletes ever). It's very different from just pumping yourself full of experimental horse hormones like they used to.

They're all on roids and they're special on top of it. There's just too much money and fame to be achieved in sports that there will always be people willing to cheat. And while roids won't make everyone a world champion they'll carry people that are already world class into world champions.

The testing windows are such that it's possible to micdose shit like EPO and HGH without a fear of getting caught. With EPO and blood doping the only issue is sports with biological passports. But you can even game that by "altitude camps", starting before you require a bio passport or increasing your levels consistently over the years but slowly enough to not raise suspicion.

Modern doping testing started as a direct response to Tom Simpson dying at the slopes of Mont Ventoux. It has never been about wanting to prevent doping, but to prevent the athletes from overdoing it and killing themselves. Because athletes killing themselves with dope is really bad for business. And sports are business after all. What's good for business is superhuman feats broadcasted right into your living room. The sports federations don't even care about doping as long as people don't get caught. And if a big name gets caught they'll somehow explain it away with contamination, a rogue doctor, masseuse, or whatever. Then once in a while some jobber gets caught and is made an example of to sell the image of clean sports and to show how doping is taken seriously.

Some sports, all of the big American sports for example have even more relaxed testing and banned substances lists than WADA has. But they still test to sell the image of a clean sport and to keep the athletes from killing themselves.
