Do you ever let you PCs pick insanely dumb armor options? I’m talking like allowing them to wear no shoes in a dungeon or wear no armor to expose their stomach? Do you devise ways to screw over their choices?

I think temperature is the biggest factor that players miss. You can't be running in a multilayered plate armor with padding in the desert, Mr Cleric.

Unless I'm running a system that has hit allocation where the details of armor are that specific, I don't care. If the game isn't going to punish them for not wearing a helmet, I'm certainly not going to houserule in an entire subsystem just to screw them over.

I've been in this hobby for over twenty years and I've never once seen a player describe how their character is barefoot or their armor leaves their belly exposed, I know you don't actually play any games but you have a weird idea of what they're like.

Depends on the system. If there aren't locational armor rules, then don't bother. Insanely dumb systems encourage insanely dumb choices. At worst, I might describe a particularly nasty hit as striking an unarmored location.
If there are rules for locational armor, then just play the game as intended. The rules will sort them out. PCs who eschew armor in systems where it matters often have compensating advantages anyways, like Natural Armor.
