Cairn is far from the aesthetic you're talking about, the 1st edition uses bog-standard public domain art, and the 2nd edition uses tumblr style fantasy art. The Israeli Jewish author also actively hates the style of old school D&D because he finds math annoying and says that Gygax was a racist fascist. Not "dungeon synth" at all.Shadowdark is closer to what you're saying, but the art still feels modern to me. The system is alright, it's basically a stripped down 5e with some DCC mechanics thrown in. The tone goes for a sort of pastiche of grim-darkness with some gonzo stuff. The lesbo author likes the old school style of D&D and made the system to recapture her childhood memories of playing AD&D 2e. It's decently dungeon-synth-ish.Dungeon Crawl Classics is a beautiful work of art, I sometimes just flip through it to look at the awesome illustrations, and buying any of their adventures is worth it just for the awesome art. I wouldn't call it gritty and dungeon-synth-ish, more crazy guitar riffs and weird psychedelic art on old fantasy books you'd find at an estate sale.Dragonslayer also has that cool black and white style grittiness to it, it's massive and over-priced but it is filled with cool old-school art. I'd recommend this over all the others I mentioned if you want to go with gritty Dungeon-synth-ish style.Also check out Labyrinth Lord and the previous editions of Swords and Wizardry.

There's an entire chunk of coffee table books where they gave up on pretending it had game mechanics and just gesture at the idea of a game that it could have been but never was. The OSR hype and the ttrpg boom is calming down a bit which is honestly nice. It'll cut down on the shovelware, the marketing and let some of the enthusiasts breath a bit.
