Ever had to say fuck it this is above our paygrade, and just bail from a dungeon?
I get that GM probably put some amount of effort into preparing it and skipping the place would put that to waste, but when the very first encounter makes it painfully obvious pushing on would be suicidal, nobody can really blame the party for doing literally anything else, right?

It happens from time to time but as forever GM I almost universally bait the party back into the dungeon somehow, for example by threatening them with consequences of not completing the dungeon swiftly, or by sweetening the deal with increased rewards, or by psyops like mentioning how a different group will claim the rewards or how their close friends/family are counting on the party clearing the dungeon etc. etc. Unless this was a minor side shit in which case I don't bother and let them skip and maybe come back later if they wish.

I ran Descent into Avernus once for my D&D group. The Dungeon of the Dead Three is notorious for being entirely unbalanced (it's intended for level 1 players but multiple mobs in the dungeon will entirely wipe a party with no issues), so I had them do some dungeons to get to level 3 or so. Even with that, they got hit with 1 fireball during an encounter and nearly everyone was put to single-digit HP or were downed from full health, so they noped the fuck out of there ASAP.
By the time they gathered a group of Fists to aid them it had been a day or so, so I had the entire dungeon clear out as the secret location was compromised. They could still pick up the relevant plot clues but they were bummed they couldn't take revenge.

That's good, it means the players are actually thinking. There isn't anything you're supposed to do beyond try to make good decisions according to your goals. If the players aren't free to decide for themselves what their characters do, there's no reason for them to show up.

Yes, we've had situations like changing upon the lair of an adult blue dragon while we're like level 3. And this is something that *should* happen, the world shouldn't all be scaled to the PCs' level like a Toddslop video game but instead make the players be cautious and scout ahead and seriously consider whether some fights are worth starting.

I introduced an enemy in a game living in the catacombs beneath the city, and after fighting it once they refuse to go back down there again. Except as potentially part of a future plan to lure another powerful enemy they are dealing with in order to make them fight each other and get rid of at least one of their problems in the process. They've mentioned that as a plan a couple of times, but not had a chance to act on that yet.

In their defense, they stuffed a grenade down the creature's "throat" and the resulting explosion didn't kill it. Which makes sense to me knowing how the creature works, but as far as they are concerned that puts it outside of their ability to handle because they were 100% sure that would kill it.

notice the presence of pluralities. Plot hooks, multiple. Rumors, not singular. Different problems in different directions.
If I tell you that there is a dragon attack in the north and a goblin invasion in the south, you have a choice of options. North, south, or a different direction entirely. If you went west and ran into the dragon anyway, that would be a quantum ogres situation. If you go west and run into bandits on the road preying upon people fleeing the dragon attack, that is not a quantum ogre situation.

When I was a fulltime wagie with a girlfriend I still managed to make dungeons for my group and if they didn't go into them then I would just move on and make another one. Probably 30 percent of my dungeon content went to waste. It is what it is. Ideas can be reused.
