The Sidereal's Rising Butterfly Prayer makes somewhat clear that it can/has happened, but that ultimately Exaltation comes down to the directions of Heaven and work of Lytek, and not random chance.

Well, the reason the compass works is supposed to be because of the properties of white jade specifically, so I wouldn't expect you to be able to do that without making an artifact. White jade is attracted to white jade is the particular property I'm talking about but it's not the same for e.g. red to red or black to black. You'd have better luck making a white jade needle that points towards sources of other kinds of jade, since it's the mix of 'resonates with earth' and 'jade works well together' that does it iirc.

In addition to the other compasses needing to be artifacts, or possibly needing weird materials to function, the Wyld shifting the borders, and moving the Poles, thanks to Wyldstorms in the Age of Sorrows would make them ever so slightly inaccurate whenever that happens. Though on that thought, I don't know if distance can really be calculated all that well when dealing with things "in" the Wyld. Hell that might be a good explanation for other Direction compasses don't work well, the Wyld constantly screws with the distance

One of the first Alchemicals did. When the Alchemical that became the Patropolis of Estasia Exalted, he regained memories of serving as a Solar in the Primordial War. The reason Estasia is so militaristic and warlike is because he believed that BECAUSE he was the only (confirmed) Twice-Exalted in all of Autochthonia, he should be first amongst equals and lead the entirety of the peoples of Autochthonia.

Are any of the Exalted able to fully heal derangements? Solars and Abyssals only seem to be able to reduce it to minor intensity.

I used this in my ExWoD game, and before that the same concept in my Mage game. Basically the way I did it was that if the soul was new the past life would not resemble the original directly resulting in Avatar the Last Air Bender style reincarnation. If the same soul ended up with the same Exaltation/Avatar (something that happened pretty often due to Destiny/the soul leaning towards the same heroism as before) they'd look exactly the same like in Inuyasha or Zelda or Yugioh where the new one looks almost just like the old one and its practically the same person in different context. I mostly focused on the rebirths of my Exalts immediately prior, who were all...I guess lets call it 'true reincarnation', where both the mortal soul and immortal exaltation are present both times.

Given how they had an abyssal in a god damn wheelchair for Essence art my theory is that they see people with disabilities being who they are because of those disabilities. Prayers to him are at the very minimum efficacious at putting people into bodies destined for Dragonblooded Exaltation. And yes, Heaven (Abstract Matters specifically) does explicitly choose who Exalts as DB. Yes, this is 1e/2e. Yes, it was changed to no longer explicitly confirm this in 3e, where it is numinous.

It's not inconsistent with the setting at large either, it's a whole thing that the Immaculate Order is much more right than wrong, and in this case that their take on reincarnation and people who act properly for their station and who are more respectable being more likely to reincarnate as Dragonblood is entirely correct even if the mechanisms behind it aren't described accurately. It ties into destiny stuff too.

Gods controlling who Exaltations go to is as dumb as gods being able to decide to stop Exaltations altogether, and largely for the sane reason. Gods being able to decide which souls reincarnate as people with good Breeding and a good chance of Exaltation, with there still being an element of chance to it as not every equally well-bred youth Exalts, is completely fine, though.
