Cesky Krumlov, Bruges, Verona are 3 of my favorites that I've visited. There are endless small villages in the Alps that are really well preserved.
The "old world" is kind of dead in Europe at least, which is assume that is what you're looking for since pic rel. Anything resembling that is economic stagnation and not really genuine in practice. Don't get me wrong there is a lot of neat "poverty" tourism left in rural Eastern Europe including the Baltics, Romania, Bulgaria, Moldova and the Balkans to a lesser extent. Prepare to be bored if you want to experience anything real. The Caucasus springs to mind, Georgia, Armenia and Azerbaijan.
Brodnica, Poland felt pretty Old World-y. It also had a fair bit of traffic.
