It's much more a case of split personality, think in those terms. The one wish willow just works. It's not good or evil. There is no inherent monkey's paw attached. You can get good or bad outcomes from it entirely dependent on the wording and what you're actually wishing for. ANY wish that influences someones behaviour even the slightest is, at best, immoral. At worst you get the events of the movie. We're shown 2 other wishes, 1 explicit and 1 implied precisely (ian and the store clerk) to demonstrate that wishes can go fine for the recipient. Ian getting shot after wasn't karma, nikki killed him. Like she killed Sarah who had made no wish.
