Probably from the person’s perspective, they feel they look more grotesque than they actually do and in some cases it can look very unnatural, with whitening of hair, eyebrow, etc. I don’t know how bad mj’s case was but he may have been shown pics of severe cases and got spooked which led him to do the treatments to just have cohesive skin color instead of looking very diseased
