Three Mile AND Chernobyl became problems because the guy in charge of running the reactor had been trained on NAVAL reactors, and when things started to go sideways they BOTH intentionally countermanded automated systems that were designed to stop anyone from doing the thing they were trying to do.
That being said, Chernobyl wasn't the only time those RBMK reactors had problems, it had just been covered up because nothing dramatic happened that couldn't be covered up.
Nuclear power is genuinely pretty safe, but you're going to have a hard time designing something that a person can't interfere with.

The big problem is that all the NIMBY's, and the public turning sour on atomic energy because most of the cold war was spent with people terrified they were going to get blasted into oblivion at any given moment.
Once nuclear proliferation became a big issue the government didn't have need to subsidize the industry to get enriched uranium or plutonium for bombs a lot of money ran out.
And the path to commission and build a NEW nuclear power plant has so much red tape and buffers built in that it costs so much extra money and so much more time that it guarantees the project will be canceled before they even get to break ground.
Modern reactor designs in general are all pretty fail-safe. You can just make a pebble-bed reactor where the individual pellets of fuel are individually so small that they couldn't get hot enough to cause a melt down in the first place.

You're a moron. It was fundamentally flawed for no fewer than three major reasons. 1) The design had positive void coefficient which caused power to spike as coolant was lost. Reactors designed to not explode lose power as coolant evaporates, but that requires enriched uranium and heavy water which are more expensive than unenriched uranium and regular water. 2) There was no containment building, again to cut costs. Because why splurge on a nuclear power plant? 3) The emergency shutoff was accidently a self-desert mechanism. Whoopsie! The Soviets knew that if fully retracted reinserting the control rods would cause a power surge because the tips of the rods were made of neutron-moderating graphite but fixing that was deemed too costly. Better to save money and let the entire country collapse in case of an accident.
