The general premise of "Yugi unlocks a shadowy alter ego that appears to battle villains" was still there, but it was much more about gaming, toys, and puzzles as a concept rather than one specific game. Hence "Yu-Gi-Oh" -> King of Games. Instead of sending bad guys to the Shadow Realm, they would instead suffer some form of karmic punishment when they lost and go insane. Sometimes they wouldn't even lose, but they'd cheat and be punished anyways. It was kind of like Kaiji meets Warau Salesman for kids. The original 1998 Toei anime is based on that part of the manga. It's also where we get the patrician's version of Kaiba.
