I didn't think this one had a real meaning behind all the trippy shit beyond making you uncomfortable and uncertain, but I think I get it now.
It's about the paranoia that inevitably haunts you when you feel deeply alienated.It's about how fast the pits of loneliness can turn you insane by making you question everything around you. The final scene where Adam sighs when he sees Helen as a this spider who reacts nervously toward him- it represents Adam seeing Helena seeing Adam as a spider, and vice versa. It's the two of them mutually recognizing that they're lonely and alienated and forever afraid of and suspicious of each other. Adam deciding to go to the underground club is about him in doing what Anthony was doing- dealing with this alienation in some probably perverted way. If you want to say it's about relationship insecurity, you can, but it's really a movie about the well known effects that emotional alienation has on people. It's why loneliness makes schizophrenics, at least as often as the other way around.

As for whether or not they're meant to be the same person, or as villaneuve said 'a fantastic event where one meets another self', is in my opinion irrelevant once you understand the actual point of the movie. You're meant to get an experience similar to what a person in this kind of state of mind has- a complete inability to see an absolute stable reality, because your unkillable paranoia holds your reality in a state of superposition.
