He died in the safe house. That's why the episode opens with him in the same position as when he got shot and funeral music is played The whole episode is just his version of the final stage before he's clipped and has to restart all over again in Melfi's office from the pilot

OP is unironically right. Anything after the cut to black is pure conjecture and not canon. You can't meaningfully say Tony died any more than you can say AJ ended up becoming a helicopter pilot.
